As a patient centered medical home, with seven locations located in an urban, culturally diverse setting, we serve over 20,000 residents of all ages. Our services include perinatal and women’s health, pediatric, adolescent and adult preventative, acute, behavioral health, optometry, and dental care. Our team is comprised of over 200 employees and providers of all specialties. Many of our employees are multilingual in Spanish, Portuguese, and/or Cape Verdean Creole.
